Based on clinical record review and interview, it was determined the Facility failed to fully disclose the risks and benefits of transfer specific to the condition of transferred patients. Failure to fully disclose the risks and benefits did not allow six (Patients #5, #8, #13, #15, #17 and #20) of six (Patients #5, #8, #13, #15, #17 and #20) patients to make an informed decision regarding the need for the transfer. The failed practice was likely to affect Patients #5, #8, #13, #15, #17 #20 and all patients that present to the Emergency Room needing transfer to another Facility. Findings follow:
A. Review of the clinical records of Patients #5, #8, #13, #15, #17 and #20 revealed no documentation of the risks and benefits specific to the condition for which the patient was being transferred.
B. During clinical record review with the Emergency Room Nurse Manager on 02/27/15 the findings of A were verified at 1030, 1055, 1153, 1153, 1200 and 1210 respectively.
C. During an interview with the Chief Nursing Officer on 02/27/15 at 1220, verified the findings of A.
