Based on record review and interview the facility failed to ensure appropriate screen for infectious disease was completed per facility policy in 2 of 10 medical records reviewed (Patient #2 and Patient #5) in a sample of 10 patient records.
Findings include:
Review of facility document dated 12/01/2021, titled, "[Facility] COVID practices stated, "Our patients are screened and tested for COVID prior to admission, as well as daily throughout their stay with us."
On 01/05/2022 at 9:00 AM, review of Patient #5 medical record revealed COVID testing was completed on 11/16/2021, 11/18/2021, 11/19/2021, 11/20/2021, 11/21/2021, 11/22/2021, 11/23/2021, and 11/24/2021. Documentation stated patient was unavailable for test on 11/17/2021. Pt. #5 tested positive for COVID on 11/24/2021 and was place in airborne and contact precaution isolation. These findings were confirmed during interview with RN H on 01/05/2022 at 11:46 AM.
On 01/05/2022 at 11:46 AM, review of Patient #2 medical record revealed COVID testing was completed on 01/01/2022, 01/02/2022, 01/04/2022. No evidence of a test was completed on 01/03/2022, no documentation was present as to why it was missed.
These findings were confirmed during interview with RN H on 01/05/2022 at 11:46 AM. RN H stated, "If the COVID test isn't done, it's passed on to PMs. It wasn't done on 01/03/2022 and was done on 01/04/2022, all adults on the unit were negative."
