Based upon observation and interview the facility failed to provide a sanitary environment in the kitchen prevent the transmission of foodborne and vectorborne communicable diseases. This deficient practice could potentially affect all inpatients, partial hospital program patients, and staff or visitors that consume food prepared from the kitchen. Findings include:

During the facility tour on 1/9/2018 between 1130 and 1200 with Staff J, K, L, Z, and AA of the kitchen area the following observations were made:
1. 11:30 a.m. live cockroaches and shed body parts of cockroaches were observed in 3 locations around the dish machine line.
2. 11:34 a.m. the door frame at then end of the dish machine line was observed to be rusted and the bottom 3" inches had deteriorated and was missing.
3. 11:37 a.m. the quarry tile base on the column near the cart wash area was damaged and the grout was deteriorated and no longer provided a cleanable surface. A section of wall tile approximately 16"x16" was also missing from this same column.
4. 11:45 a.m. the quarry tile base near the speciality line was damaged and the grout was deteriorated. The fiber reinforced panel wall covering at this same location was loose and no longer sealed with caulk. These conditions no longer provided a cleanable surface.
5. 11:56 a.m. dirt and debris were observed under the mobile shelving units in the deep freezer.
6. 11:59 a.m. dirt and debris were observed under the mobile shelving units in the paper dry storage area.

All the observations were confirmed by Staff J, K, L, Z, and AA during the facility tour.

Additionally, Pest sightings logs for October 2017 through January 2018 were reviewed on 1/10/18 at approximately 1430 with Staff A, Staff V and Staff U. 36 sightings of cockroaches were documented between 10/1/17 and 1/9/18, the majority of which occurred in the kitchen. Less than 10 sightings of rodents were noted during this time, either in staff lockers or in the kitchen. Staff A reported that the facility had a problem with cockroaches and rodents since July of 2017, which she attributed to the fact that the City of Detroit had dug up the adjacent main road and sewers in order to install light rail tracks. Staff A provided documentation that the pest control contract was upgraded, and drain one-way devices were installed in order to deal with the ongoing problem. Staff U reported that quarterly summary reports and environmental analysis reports from the pest control contractor were available at an additional cost but access to the information had not been approved. Staff U stated that the facility was aware that the problem was ongoing, but did not track or report numbers of sightings or response to pest control measures implemented, because of lack of access to pest control contractor data.
