Based on observations, interview and record review the Critical Access Hospital (CAH) failed to ensure 1 of 1 carbohydrate controlled diet (CHO) was prepared as prescribed by the physician. In addition, the CAH failed to provide adequate sanitation in the kitchen, where 1 of 1 wall mounted fan was heavily soiled.

Findings include:

In review of the P2's medical record there was a physician's order dated 8/29/11 for a "CHO" diet. The dietary staff failed to prepare the patient's diet following the CHO diet.

During a meal observation at 11:50 a.m. on 8/31/11 with the Certified Dietary Manager (CDM) present, cook-A prepared Patient 2's (P2) plate with a serving of baked beans. P2 was on a CHO diet. The "Spring/Summer 2011-IV Wednesday" dietary menu, specified "no beans" on the CHO diet. When cook-A was questioned about the serving at that time, both the CDM and cook-A agreed an error had been made and the beans were removed from the P2's plate before it was delivered.

In a second observation with the CDM present at 7:50 a.m. on 9/1/11, cook-B placed a blueberry muffin on the meal tray for P2. The "Spring/Summer 2011-IV Thursday" dietary menu, directed the cook to serve whole wheat toast with sugar free jelly instead of the blueberry muffin. When cook-B was questioned at that time, both the CDM and the cook agreed that an error was made and corrected the error prior to delivering the tray.

During the initial kitchen tour at 9:45 a.m. on 8/31/11, and a subsequent observation at 7:45 a.m. on 9/1/11, a fan was observed to have been mounted on the wall directly above the clean dish area which had a heavy accumulation of black debris on the blades and on the fan's grill casing. The CDM agreed the fan was very dirty and should have been cleaned especially because it was blowing on the clean dishes.

A review of the kitchen's "Cleaning Weekly" schedule, undated, revealed there had been no record of cleaning the fan. When interviewed at 9:40 a.m. on 9/1/11, the CDM stated she had missed listing the fan on the Weekly Cleaning schedule.
