Based on review of policy, observation, and interview, it was determined that the hospital failed to ensure that 1 of 1 exit door to the East Parking lot was closed and secured, potentially affecting the safety and security of all patients (67), staff, supplies and equipment.
Finding include:
1. On 7/18/12 the Hospital's policy titled, "Management Plan for Security (8/10)" was reviewed and included, .."IV. B... appropriate and timely actions is taken to prevent crime, injury, to property loss."
2. On 7/18/12 at approximately 8:45 AM during an observational tour, accompanied by the Senior Vice President of Support Services, the Hospital the exit door that leads to Hospital's East parking lot, was propped open, allowing unauthorized entry.
3. During the tour at 8:45 AM, the Senior Vice President of Support Services acknowledged the above finding during an interview.

A. Based on observation and interview, it was determined that the Hospital failed to ensure that 2 of 2 Crashcarts and 1 of 2 Suction Machines on the Medical Surgical Telemetry Unit, had scheduled preventative maintenance potentially causing harm or injury to all the 37 patients in the Telemetry Unit.
Findings include:
1. On 7/18/12 during an observational tour of the Hospital's Medical Surgical Telemetry Unit, the following was observed:
-2 of 2 Defibrillators had expired preventative maintenance stickers, Defibrillator #1 had an expiration date of 4/1/12, and Defibrillator #2 had an expiration date of 7/1/12.
-1 of 2 Suction Machines had an expired preventative maintenance sticker, dated 6/1/12.
3. On 7/18/12 during the tour, the Director of Facilities also observed and acknowledged the above findings.
