Based on clinical record review, review of policies and procedures and interview, it was determined the Facility failed to obtain consents for three (#2, #3 and #9) of ten (#1-10) patients admitted to the Facility. The failed practice did not assure patients were informed of or agreed to their plans for care and had the potential to affect all patients admitted to the Facility. Findings follow:
A. Review of the clinical records for Patients #2, #3 and #9 revealed no evidence a consent form was signed or made part of the clinical record.
B. Review of Policy, AD-1.5 revealed Patients have the right to receive from their physicians information needed to give informed consent prior to the start of any procedure and/or treatment.
C. During interview with the Quality Management Analyst on 08/08/14 at 1220, she confirmed the findings.
