Based on observation during the life safety survey completed on 02/25/2010, the hospital did not maintain corridor doors to be free from obstructions to closing and positive latching as required.

Findings:

1. Positive latching hardware on two of two south tub room storage doors and on Room 1050 was observed to be compromised by tape being placed over the latching mechanisms. This was corrected during survey.

2. The ER corridor access door was observed to be held open by a rubber floor wedge. The wedge was removed during survey.

These observations were made in the presence of the Director of Nursing and the Director of Plant Operations.

Based on observation during the life safety survey completed on 02/25/2010, the hospital did not maintain exit discharge walkways to be free from obstructions as required.

Findings:

The north long term care exit discharge path of egress to a public way was observed to be obstructed by an excessive accumulation of snow and ice.

Findings:

The main electrical panel room also contains an auxiliary fire control panel. This room was not protected by an integrated smoke detector.

Based on observation and interviews during the life safety survey completed on 02/25/2010, the hospital does not have documentation to verify that weekly and monthly tests of the emergency generator are being conducted in accordance with NFPA 99.

Findings:

The hospital could not provide documentation that weekly inspections and monthly load tests of the emergency generator are being conducted. In an interview conducted on 02/25/2010, the Director of Plant Operations stated that the remote annunciator panel indicated the weekly exercise of the generator, but does not provide documentation of the weekly and monthly load tests as required.
