Based on observation and interview, it was determined the facility failed to maintain one set of smoke barrier separation doors, affecting two of six smoke compartments.
Findings include:
Observation on August 19, 2015, at 10:12 a.m. revealed the smoke barrier separation doors closest to 2093A did not fully close when released from magnetic hold-open devices.
Interview on August 19, 2015, at 10:13 a.m. with Facilities Manager confirmed the smoke barrier doors did not fully close when released.
