Based on observation and interview, the facility failed to maintain the means of egress as required.
Findings include:
On September 24, 2014 the following doors were equipped with two locking/latching devices where two releasing operations were required to operate the door. Doors in the means of egress are required to be operable with not more than one releasing operation.
Deadbolt locks were added in the following locations requiring an additional releasing operation:
Lakota Lodge
Room 127 (double key), 123, 125, 105, 116, 103
Existing originally installed locks are acceptable as an existing condition.
The Facility Manager was present when the deficiency was identified.
Failure to maintain the means of egress as required increases the risk of death or injury due to fire.
The deficiency affected six of numerous doors in the building.

Based on observation and interview, the facility failed to provide an emergency lighting as required.
Findings include:
On September 24, 2014 the generator providing power for emergency lighting did not have a remote manual stop outside of the room housing the prime mover (diesel motor) or elsewhere on the premises where the prime mover located outside of the building as required.
The Facility Manager was present when the deficiency was identified.
Failure to provide an emergency electrical system as required increases the risk of death or injury due to fire.
The deficiency affected one of numerous components of an emergency electrical system.

Based on observation and interview, the facility failed to install an automatic fire sprinkler as required.
Findings include:
On September 24, 2014, the automatic fire sprinkler inspectors test connection did not terminate in a smooth bore corrosion resistant orifice giving a flow equivalent to one sprinkler of a type having the smallest orifice installed on the particular system. The inspector test connection terminated at the exterior of the building with what appeared to be 1.5 inch steel pipe.
The Facility Manager was present when the deficiency was identified.
Failure to install a sprinkler system as required increases the risk of death or injury due to fire.
The deficiency affected one of numerous components of an automatic sprinkler system.
