During the tour of the facility with staff M at 11:50am on 05/21/12 it was determined that there was an unsealed penetration in the corner of the boiler room near the electrical panels of approx. 6 inches.


In interview with staff M this items was confirmed on 05/21/12 at 11:55am.

Based on observation and staff interview it was determined that the facility failed to ensure that the kitchen hood system complied with NFPA 96, this could place the employees of the kitchen at risk.

During the tour of the facility on 05/21/12 at 11am with staff M it was determined that the kitchen hood system had limited combustible sheetrock that was in contact with the hood system on the left side at the ovens.
