Through observation during the survey, conducted July 10, 2013, it was determined that the facility failed to maintain the doors to the corridor. 19.3.6.3.2


During the walk through of the facility with staff:

1) one leaf to the double doors to the electrical room (south corridor) was affixed with a manual thumb latch. Doors to the corridor are required to positively latch.

2) The communication closet door did not latch into the frame when in the closed position.

Through observation during the survey, July 10, 2013, it was determined that the facility failed to maintain the smoke barrier walls. 19.3.7.3

During the walk through of the facility with staff, Several unsealed penetrations were found above the ceiling in the rehabilitation room, and above ceiling in the recreation room.
