Based on observation and interview, the facility failed to provide separation of hazardous areas from other compartments. This facility has 25 certified beds and at the time of the survey the census was 3.
Findings include:
1. Observation and interview on 10/1/12, revealed the corridor door to the Kitchen (by the desk area) did not close and latch properly when tested.
2. Observation and interview on 10/1/12, revealed the corridor door to the Clean Storage (across from Room 107) did not close and latch properly when tested.
Administrative Staff A and Maintenance Staff A verified these observations.

Findings include:
Observations, record review and interview on 10/1/12, revealed the facility failed to have the commercial cooking suppression system inspected every six months. The last two inspection performed were dated 12/12/11 and 8/20/12. Maintenance Staff A verified this observation.
