Based on observation, review of policy, an interviews with the facility staff; the facility fail to maintain a safe environment; as the sink at the nurses station had brownish, rusty colored stains on the sink backboard and window sill, there was a 3 inch opening in the caulking, and a tiny spider was observed on the sink.
Findings were:
During a tour of the nursing station at the facility on the morning of 4/1/14 the sink was observed to have brownish, rusty colored stains on the sink backboard and window sill, there was a 3 inch opening in the caulking, the caulking was blackish color, and a tiny spider was observed on the sink.
Review of policy 82-32, effective 10/1/2013 environmental services personnel will clean the nurses' station on a daily basis. Clean sinks and exteriors of all dispensers as per procedure: CDC-10, spot clean walls, doors and partitions as needed.
In an interview with staff # 6 and #4 during the tour of the nurses station on the morning of 4/1/14 the findings were confirmed.
