PATIENT RIGHTS: INFORMED CONSENT

Based on review of records and interview, 1 (Patient #7) out of 3 patients (Patient #7, #8, and #9) who had been admitted under voluntary status were given psychotropic medications without signing appropriate consents informing the patient of the medication, risks, side effects, and benefits.

Findings include:

On 2-8-2018, current patient charts were reviewed with Staff #5. Review of Patient #7's charts revealed no medication consents on the chart. Staff #5 was asked to locate the medication consents on the charts. Staff #5 was unable to locate the consents on the chart. Staff #5 asked Staff #9, medication nurse, if she had the consents in the medication room. Staff #9 stated, "Maybe". Staff #9 entered the medication room and then returned stating that she did not have any consents in the medication room.


Review of Patient #7's chart revealed he was a 50-year-old patient, admitted on 2-1-2018. Home medications of Ativan, Risperidone, Lithium, Aricept, Remeron, and Restoril were continued at previous dosage. The home medication dosage of Paxil was discontinued. A new order for Paxil at a different dosage was initiated.

The Risperidone dosage was changed on 2-2-2018. Neudexta was ordered at this time also.

No medication consents were found.

An interview was conducted with Staff #9. Staff #9 confirmed that she had given medications without looking to see if consents hed been properly signed.
