Based on medical record review, it was determined the facility failed to allow the patient to make an informed decisions regarding their care.
The findings include:
Record review revealed Patient #1 was admitted to the hospital on 5/10/12 for severe dizziness, depression, hypertension and diabetes.
On 5/11/12 the physician charted the patient talked of feeling depressed and ending her life. The physician ordered a psychiatric consult and placed the patient on 1:1 observation. During the psychiatric consult the patient denied she had expressed suicidal ideations but was very angry, upset and seemed to be depressed. The patient started refusing all treatments, tests and medications around 2:00 PM on 5/11/12. The physician on 5/12/12 charted awareness of the patient's refusal of all treatment and medications and would "not attempt to force tx on patient."
Nursing notes of 5/12/12 revealed at 1415 the patient was very agitated and still refusing all treatments and medications. The physician was notified and order was received to give an intramuscular (IM) injection. Nursing called the house supervisor and documented they would need male assistance to administer the IM injection.
Charting on the Medication Administration Record (MAR) show Ativan 2 mg given IM at 1417.
The patient continued to refuse all medications and treatments on 5/13/13 but took some medications on 5/14/13 before she was discharged.
