Based on observation, record review and interview the facility failed to ensure a safe environment by allowing children to move furnture next to the nursing station so that they could take contraband from within the nursing station.
On 10/18/22 a video of patients in the adolescent unit 6 on 10/7/22 at 19:14 (patient A) place a chair against the nursing station and and 3 other unkown adolescent patients used the chair to climb on and obtain items (contraband) from within the nursing station, without the nursing staff being aware.
At the time of viewing with Risk Manager he acknowledged that the patients should not be using the chair to climb and access the nursing station.
Interview with 2 nurses on unit five employee B and C on 10/18/22 at 1330 revealed that the patients on unit 5 are not allowed within five feet of the nursing station. nor is any furniture allowed against the wall of the nursing station. This prevents the children from trying to enter or grab stuff from the nursing station.
