The hospital reported an average census of 1.4 patients per day with 30 records reviewed. Based on observation, policy review, and staff interview the Infection Control Officer failed to develop and implement policies and procedures to ensure equipment remained intact to prevent infections for two of two operating room (OR) beds in accordance with accepted standards of practice.
Findings include:
- Association of periOperative Registered Nurses (AORN) directs " ...Mattresses and padded positioning device surfaces (eg, OR beds, arm boards, patient transport carts) should be moisture-resistant and intact ...Damaged or worn coverings should be replaced ... "
- Observation on 11/26/13 at 2:00pm revealed the operating bed in OR #1 with two open areas approximately 3x3 inches in diameter. The open areas on the bed were covered with Opsite (a waterproof film used for covering wounds).
Staff A, Infection Control Officer, interviewed on 11/27/13 at 8:30am confirmed the operating bed in OR #2 also had two open areas approximately 3x3 inches in diameter. The open areas on the bed were covered with Opsite (a waterproof film used for covering wounds).
Staff A, Infection Control Officer, interviewed on 11/27/13 at 8:30am acknowledged the hospital follows AORN guidelines and that the operating room beds in OR #1 and OR #2 were not intact.
