Based on observation during a tour with staff on 11/22/2011, in the only operating room, the dual arm lighting device had been replaced, however the faceplate that affixes to the ceiling had not been secured to the ceiling leaving an open area that could not be cleaned compromising patient safety.
Findings were:
Observation in the only surgery room revealed that the faceplate and was about four inches away from the ceiling hole where the lighting was attached. Additionally the ceiling in the area where the faceplate was unsecured from the ceiling, had three separate colors of paint and three large, peeling cracks in the paint and plaster. This area was directly above the surgery table in the operating room, not ensuring the safety of patients from possible plaster and/or paint pieces falling. Interviews with the Nursing Director and administrative staff confirmed the above findings.
