Based on interviews and documentation review, it was determined the Hospital had not (yet) developed and implemented a Corrective Action Plan related to its Internal Investigation of Patient #1's care/stage III pressure ulcer.
Findings included:
A review of the Hospital Internal Investigation related to Patient #1's care/stage III pressure ulcer revealed it identified opportunities for improvement related to BiPAP masks and physician documentation (a procedure note was not written following debridement of the ulcer and his/her Discharge Summary was incomplete) and a nursing education need related to skinfold assessment.
The Hospital was considering the use of a different BiPAP mask but had not (yet) come to a decision.
Corrective actions related to the physician documentation issues and/or the nursing education need related to skinfold assessment had not (yet) been developed and implemented.
