Interior Nonbearing Wall Construction

Based on observation and interview, the Hospital failed to limit combustible in two newly built interior walls per NFPA 101, Life Safety Code, 2012 Edition, Section 19.1.6.4
Buildings of type I or II interior walls shall be constructed of noncombustible or limited combustibles materials.
Findings:
1. Combustible materials were used to build and support a wall in the Emergency department receptionist area.
2. Combustible materials were used to build and support a wall in the Emergency department exit corridor leading to the Main Hospital
receptionist area.

These findings were confirmed with the Safety Officer and Project Manager present.

Means of Egress - General

Based on observation and interview, the Hospital failed to maintain exit corridors free of obstructions per NFPA 101, Life Safety Code, 2012 Edition, Section 19.2.1. & 7.1.10.1, 7.2.1.4.2
Findings:
On October 5, 2022, between 10:00 am and 2:00 pm this surveyor accompanied by the facility Safety Officer and Project Manager did observe Stored equipment in the ED smoke compartment corridors and an egress door not swinging in the means of egress.
Means of egress shall be continuously maintained free of all obstructions or impediments to the full instant use in the case of fire or other emergency.
Doors are required to swing in the means of egress

1. Computers on wheels stored in Emergency department corridor.
2. Chairs and stacked chairs found in the Emergency department corridor.
3. 32- gallon Soiled linen carts throughout corridors in the Emergency department.
4. Food carts stored in corridor of the Emergency department.
5. Wall and door erected perpendicular between the Emergency department Smoke barrier wall double doors and the Hospitals Maine reception does not swing in the means of egress when exiting from the Main Hospital reception area.

These findings were confirmed with the Safety Officer and Project Manager present.

Aisle, Corridor, or Ramp Width

Based on observation and interview, the Hospital failed to maintain exit corridor widths per NFPA 101, Life Safety Code, 2012 Edition, Section 19.2.3.4 exceptions 1-5
Findings:
On October 5, 2022, between 10:00 am and 2:00 pm this surveyor accompanied by the facility Safety Officer and Project Manager did observe corridor width reduction from 8' to approximately 4', chairs and benches were found unsecured on both sides of the corridor walls in the Exit corridor leading to the Main Hospital reception area when exiting the Emergency department.

Per interview with the Safety Officer and the Project Manager the reduction in width of the corridor was due to the hospital using this corridor to create a new Emergency depart waiting room.

1. Unsecured Chairs and benches found on both sides of the exit corridor reducing the corridor width to less than the allowed 6'.

These findings were confirmed with the Safety Officer and Project Manager present.

Exit Signage

Based on observation and interview, the Hospital failed to maintain exit signs per NFPA 101, Life Safety Code, 2012 Edition, Section 19.2.10.1 and 7.10
Exit signs are required to be displayed in accordance with 7.10 and have continuous illumination.

Findings:
1. No Exit sign above 2- hour fire rated doors in Main Hospital reception area.
2. Exit sign above emergency room double doors shall meet 7.10

These findings were confirmed with the Safety Officer and Project Manager present.

Corridors - Construction of Walls

Based on observation and interview, the Hospital failed to construct corridor walls with a least ½ hour fire resistance rating per NFPA 101, Life Safety Code, 2012 Edition, Section 19.3.6.2
Findings:
On October 5, 2022, between 10:00 am and 2:00 pm this surveyor accompanied by the facility Safety Officer and Project Manager did observe and confirm during interview: walls were erected to create expected summer patient overflow for the hospital. One wall created an additional patient care area and the other a waiting area.

1. Two new corridor walls were created using light aluminum framing and non-typical plastic material fasten together. Exposed wooden studs and strapping were found securing the walls to the existing hospital structure.

These findings were confirmed with the Safety Officer and Project Manager present.

Corridor - Doors

Based on observation and interview, the Hospital failed to install corridor doors per NFPA 101, Life Safety Code, 2012 Edition, Section 19.3.6.3
Finding:
On October 5, 2022, between 10:00 am and 2:00 pm this surveyor accompanied by the facility Safety Officer and Project Manager did observe:
1. Door located in newly built corridor wall in the Emergency Department is required to be of 1 ¾" thick, solid-bonded core wood.
2. Door located in a newly built wall in the exit corridor located between the Main Hospital Reception area and the Smoke Barrier doors to the Emergency Depart is required to be of 1 ¾" thick, solid-bonded core wood.

These findings were confirmed with the Safety Officer and Project Manager present.

Soiled Linen and Trash Containers

Based on observation and interview, the Hospital failed to keep soiled linen and trash containers in a hazardous area per NFPA 101, Life Safety Code, 2012 Edition, Section 19.7.5.7


Findings:
1. Soiled linen cart approximately 4' x 5' x 6' found a third full stored in corridor near Ambulance exit.
2. Garbage/trash cart approximately 4' x 5' x 6' found full stored in corridor near Ambulance exit.

These findings were confirmed with the Safety Officer and Project Manager present.
