Based on observations during the facility walk-through inspection of the facility with the maintenance supervisor on 05/01/2014, it was noted that the rated door between the corridor and central supply was held in the open position without an automatic door closer that would automatically close the door upon activation of the fire alarm system. Th e supply room is a hazardous area in excess of 100 sq. feet in area .

Based on review of records and discussions with the maintenance supervisor duiring the facility walk-through on 05/01/2014, it was noted that the facility had not been documenting that the emergency generator was being tested under load conditions (30% minimum) for at least 30 minutes, monthly as per NFPA 99, 3.4.4.1.
