Based on observation and interview, the provider failed to maintain the width of corridors (clear and unobstructed) that served exit access by keeping two pieces of rehabilitation equipment in the egress corridor. A set of parallel bars and an exercise bicycle were kept in the corridor outside the double-door entrance to the physical therapy department. Findings include:
1. Observation at 1:30 p.m. revealed a set of parallel bars and an exercise bicycle were kept in the corridor outside the double-door entrance to the physical therapy department. The parallel bars and exercise bicycle restricted the corridor width by 42 inches. Interview with the administrator at 2:30 p.m. confirmed that condition.

Based on observation and interview, the provider failed to maintain a secured location for the outside oxygen cylinder storage and oxygen gas manifold. The oxygen cylinders were stored in an unlocked metal shed outdoors. The oxygen gas manifold system was also located outdoors adjacent to the exterior wall but was not in a secured enclosure. The provider must comply with the National Fire Protection Association (NFPA 99), Standard for Health Care Facilities, section 4-3.112(a) Storage Requirements. Findings include:
1. Observation at 10:30 a.m. revealed the oxygen cylinders were stored in an unlocked metal shed outdoors. The oxygen gas manifold system was also located outdoors adjacent to the exterior wall but was not in a secured (locked) enclosure. Interview with the administrator at 3:30 p.m. confirmed those findings.
