PATIENT RIGHTS: INFORMED CONSENT

Based on review of documentation and interview, it was determined that minor patient # 1's parents were not informed when patient had an injury.
Findings were:
Patient # 1 sustained documented injuries on 9/7, 9/8, 9/14 and 9/15/20. There is no notification of the parents for any of those incidents noted in the patient record.
The lack of documented notification was confirmed by the Director of HIM on 4/5/2021.
