Based on review and interview the governing body failed to ensure policies were administered as to provide quality of health in a safe environment by not following their grievance policy.
Findings:

A review of the 1/24/19 grievance revealed patient #9 presented to the emergency department on 01/24/19 and left (AMA) against medical advice. Continued review revealed Patient #9 left because he was not happy with the comments Staff J (physician assistant) made about her primary provider. Patient #9 grievance addressed that Staff J told her that she did not approve of her primary provider's plan of care. Further review revealed the critical access hospital did not document that an investigation was conducted.

A review of the critical access grievance policy "Version Effective" 07/20/17 revealed "Directors/Managers will investigate the grievance, facilitate a written resolution within the specified timeframes and document the response".

On 07/02/19 at 5:15 PM staff B stated she was the quality coordinator and risk manager. Staff B also stated she was familiar with the grievance and confirmed that the hospital did not do a thorough investigation of Patient #9's grievance.
