Based on observation, it was determined that the facility failed to ensure interior finishes have a flame spread rating of Class A or B.

Findings include:

1. On 4/2/14 at 10:25 AM, in the presence of Staff #76, the chapel, which was not fully sprinklered, had wall surfaces that were less than a Class B rating.

Based on observation, it was determined that the facility failed to ensure exit lights are continuously illuminated.

Findings include:

1. On 4/2/14 at 11:45 AM, in the presence of Staff #76, the exit light at room #130 was not lit.

Based on observation, it was determined that the facility failed to ensure the sprinkler system was continuously maintained in reliable operating condition.

Findings include:

1. On 4/2/14 at 10:45 AM, in the presence of Staff #76, in storage Room #27, two boxes of supplies were stored within 18" of the sprinkler system.
