Based on observation and interview, this facility is not assuring that one of two fire barriers are free of penetrations that compromise the fire-resistance rating of the walls and allow the passage of smoke and fire to another smoke zone. It was determined the facility failed to maintain the 2 hour fire resistive rating of the fire barrier. This deficient practice affects all occupants of the building, including staff, visitors and residents in two of three smoke zones. This facility has a capacity of 25 with a census of 7 residents.
Findings include:
Observation and interview on 11-27-12 at approximately 10:32 a.m., the two hour fire wall separating the Nursing Facility from the Hospital above the ceiling tiles contained a 1/2 gap around recently installed fire alarm wires.
Maintenance Staff (A) confirmed this observation.

Based on observation and interview, the facility did not assure that the fire alarm system is in accordance with NFPA 72, and chapter 9.6.4 of NFPA 101 by ensuring that the fire alarm breaker is mechanically protected. This deficient practice affects all occupants of the building, including staff, visitors and residents. This facility has a capacity of 25 with a census of 7.
Findings include:
Observation and interview on 11/27/12, the facility failed to provide a properly maintained fire alarm system. The fire alarm breaker located in electrical panel LLCL breaker #10 was not secured with a mechanical lock to assure that the breaker is not inadvertently shut off. All occupants would be directly affected by the deficient practice.

Based on observation and interview, the facility is not maintaining the sprinkler system in accordance with the 1998 edition of NFPA 25, 2-2.1.1, by ensuring that sprinkler heads are free of foreign material. This can effect the operation of the heads by obstructing spray patterns, delay the response time or even cause the heads to be inoperable which can compromise the effectiveness of the fire suppression system and place occupants at risk of injury in the event of a fire. This deficient practice affects all staff in one of three smoke zones. The facility had a capacity of 25 and a census of 7 at the time of survey.
Findings include:
Observation and interview on 11-27-12 at approximately 10:19 a.m., revealed that in the Kitchen one of twelve sprinkler heads on the ceiling was coated with dust that covered nearly one-half of the quick response bulb. The sprinkler head was located next to the north east corner of the Kitchen.
Maintenance Staff (A) verified this observation.

Based on observation and interview, it was determined the facility failed to maintain the buildings electrical wiring system in accordance with National Fire Protection Association (NFPA) Standard 70, National Electrical Code, 1999 edition, placing the Staff and Residents of the facility at risk in the event of a fire. The facility had a capacity of 25 and a census of 7 at the time of the survey.
Findings Include:
1. Observation and interview on 11/27/12 at 9:15 a.m., revealed the facility failed to maintain the electrical system in the Storage Room Corridor. The electrical panel NLPT did not contain a directory indicating the location of the panel breakers.
2. Observation and interview on 11/27/12 at 9:22 a.m., revealed the facility failed to maintain the electrical system in the Physical Therapy Corridor. The electrical panel in the I.T. room #3 contained combustible cardboard boxes in-front of the panels.
3. Observations and interview on 11/27/12 at approximately 10:04 a.m., revealed the facility failed to provide a Ground Fault Circuit Interrupter electrical outlet in the south wall of the of the Facility Break room next to the sink.
4. Observations and interview on 11/27/12 at 10:25 a.m., revealed the facility failed to maintain the electrical system in the Record Storage room. The storage room contained a drop cord hanging from the ceiling light used for a dehumidifier.
