Fire Alarm System - Testing and Maintenance

Based on observation and interview it was determined that the facility failed to test and maintain the fire alarm system, on one of one systems.

Findings include:

Observation on May 1, 2019, at 12:20 p.m., revealed the Acute Care Nurse Station fire alarm remote annunciator panel, had a damaged digital display screen that was not readily readable.

Sprinkler System - Maintenance and Testing

Based on observation and interview it was determined that the facility failed to install and maintain automatic sprinklers, in 20 of more than 75 rooms.

Findings include:

Observation on May 1, 2019, between 12:30 p.m. and 1:10 p.m., revealed patient rooms one thru 20 had automatic sprinklers installed within twelve inches of overbed mural frames, obstructing the automatic sprinkler.

Interview with the director of facilities on May 1, 2019, at 1:10 p.m., confirmed patient rooms one thru 20 had automatic sprinklers obstructed by over bed mural frames.

Electrical Systems - Essential Electric Syste

Based on document review and interview it was determined that the facility failed to test the emergency generators per NFPA 110-8.4.9, on three of three emergency generators.

Findings include:

Document review on May 1, 2019, at 9:30 a.m., revealed the facility lacked documentation verifying the Level One emergency generators had been exercised for four continuous hours in the previous 36 months.

Observation on May 1, 2019, at 12:05 p.m., revealed the gift shop had an extension cord plugged into an extension cord, in use, located on the center display stand.
