Based on observation, staff interview and review of hospital policy it was determined the hospital failed to maintain a clean hospital environment. The hospital failed to maintain a clean patient commode in two (2) out of three (3) bathrooms that were verified as being clean and awaiting a patient admission. The failure to maintain a clean bathroom after the discharge of a patient has the potential to increase the risk of infections to all patients admitted to the hospital.

Findings include:

1. A tour was conducted of unit eight (8) west with the Director of Performance Improvement (PI) on 6/15/15 at 12:40 p.m. Observation of rooms verified as being clean and empty by Housekeeper #1 revealed room eight hundred and eighty-four (884) had dried feces on the top of the commode seat.

2. An interview conducted with Housekeeper #1 on 6/15/15 at 12:45 p.m. revealed she cleaned the room on 6/15/15 at 12:15 p.m. after a patient was discharged.

3. The Director of PI concurred with these findings.

4. Observations on 6/17/15 at 11:00 a.m. of patient rooms verified as being clean and empty by Housekeeper #2 revealed room eight hundred and thirty-four (834) had dried feces on the inside of the commode seat.

5. An interview conducted with Housekeeper #2 on 6/17/15 at 11:05 a.m. revealed she concurred with the findings of stool on the inside of the commode seat and she later re-cleaned the commode.

6. An interview was conducted on 6/17/15 at 11:15 a.m. with unit eight (8) west's Nurse Manager. She verified the room had been cleaned on 6/17/15 at 12:05 a.m. and the room was empty and awaiting an admission.

7. Review of the policy titled, "Department of Housekeeping Daily Room Cleaning", with a last review date of 10/14, states, in part: "Bathroom cleaning toilet seat top and bottom." It was further revealed step number seven (7) is to stand in the doorway and inspect an overall view of the room.
