Based on observation and interview, the hospital failed to ensure that the storage of drugs were in accordance with professional principles when an expired medication was available for patient use in the operating room. This failure could expose a patient to unsafe conditions, due to ineffective medication potency.
Findings:
During an inspection of the surgery departments medication storage refrigerator with the Chief Nursing Officer- Acute (CNOA) and the Operating Room Manager (ORM) on 8/21/12 at 9:25 am, one 10-ml (milliliters) vial of rocuronium bromide (a muscle relaxer used in anesthesia) with the expiration date of 4/2012, was found among the other active medications available.
CNOA and ORM acknowledged the above finding. ORM confirmed that the identified medication was expired and according to the hospital's policy and procedure regarding medication storage, it should not have been available for patients use.
