Based on record reviews and interview, the hospital failed to ensure that each patient was informed of his/her visitation rights including any clinical restriction or limitation on such rights and the reason for the clinical restriction or limitation for 5 of 5 medical records reviewed (Patient #1, #2, #3, #4, #5).
Findings:

Review of the hospital's policy ADM 066 titled, "Visitation" revealed in part: It is the policy of this hospital for all patients to have the right to receive visitors. This right can only be limited on an individual basis by a physician for reasons of psychiatric necessity or security. In the event the Visitation is limited: 1. The physician fully explains to the patient the reasons for limiting visitation in the medical record. The entry is signed and dated and an order for visitation limitation is written.

Review of the hospital's "Patient Handbook" revealed in part: page 6 "Visitation and Telephone Calls" - Visiting hours vary depending on the unit. If, however, your family cannot visit during these times, please notify your counselor so that special arrangements can be considered during the appropriate time.

Review of medical records for Patients #1-5 failed to reveal the patients were informed that, due to the COVID pandemic, visitors were not allowed in the hospital.

In an interview on 04/25/2023 at 10: 40 a.m., S1Administrator confirmed the findings and acknowledged their visitation policy and the Patient Handbook were not updated to reflect that visitation was suspended due to the current pandemic.
