Based on review of facility documents and medical records (MR), it was determined that the facility failed to ensure that all medical record entries were timed for 29 of 30 medical records (MR1, MR2, MR3, MR4, MR5, MR6, MR7, MR8, MR9, MR10, MR12, MR13, MR14, MR15, MR16, MR17, MR18, MR19, MR20, MR21, MR22, MR23, MR24, MR25, MR26, MR27, MR28, MR29 and MR30).
Findings include:
Review of the Warren State Hospital Procedure Manual No. 1859, rev. February 2011 revealed, "Procedure: ... 3. Every entry must be fully dated and timed using military or 24 hour time (Ex: 2/1/11 1430.)"
1. Review of (MR1, MR2, MR3, MR4, MR5, MR6, MR7, MR8, MR9, MR10, MR12, MR13, MR14, MR15, MR16, MR17, MR18, MR19, MR20, MR21, MR22, MR23, MR24, MR25, MR26, MR27, MR28, MR29, and MR30) revealed entries on one or more of the following forms and/or documents without the time of the entry documented; Psychiatric Evaluation, Suicide Screening Form, Suicide Risk Assessment, Falls Assessment, Dysphasia Risk Assessment, SRS Unified Assessment (sections 1 through 6), Spiritual Assessment, Psychology Re-Evaluation, Admission Note by Psychiatrist, Master ITP [Interdisciplinary Treatment Plan], Osteoporosis Screen, ITP Reviews, Admission Risk Profile, Patient Personal Safety Self-Assessment and lab reports.
