Based on a review of the Medical Staff Rules and Regulations and medical record review, the facilty did not ensure the medical staff were compliant with the rules and regulations. Specifically, telephone/verbal orders were not authenticated by the provider for 4 of 15 records reviewed.(Patients #12-15)
Findings include:
Review, on 5/29/14, of the Medical Staff Rules and Regulations, dated 1/31/14, revealed that telephone/verbal orders are to be authenticated by the provider within 48 hours.
Review of medical records on 5/28/14 revealed telephone/verbal orders were not authenticated in 4 of 15 records. Examples include:
-Patient #12: Five orders from 4/06-4/11/14 were not authenticated.
-Patient #13: Two orders from 5/07 and 5/08/14 were not authenticated.
-Patient #14: Five orders from 4/05-4/14/14 were not authenticated.
-Patient #15: Four orders from 3/10-17/14 were not authenticated.
