Based on record review and interview, the facility failed to ensure any providers had approved admitting privilieges to swing bed status for 7 (#16-22) of 7 providers reviewed. This failed practice may curtail the number of physicians admitting patients to swing bed status at this hospital and could place patients at risk for a decline in health due to a lack of continuity of care.
The findings are:

A. Record review of Staff # (S#16, 17, 18, 19, 20, 21,22) credential files revealed no Swing Bed privileges listed on the "Request for Privileges" (Approved 9/24/15) form. There was no approval for privilages to admit any patient to Swing Bed status.

B. On 11/6/18 at 8:46 am, S#6 confirmed " none of their privileges say Swing Beds. They (S#16, 17, 18, 19, 20, 21,22) have privileges for their speciality only". Meaning that none of the providers at the hospital.

C. On 11/5/18 at 5:15 pm during interview, Staff #2 (S#2), stated "the policies are still in process of writing they have not been approved by the governing body." Confirming that there were no current policies in place for priveleges to Swing Bed Status.
