Based on observation and interview with staff, the facility failed to test and maintained the fire alarm system accordance with NFPA 70 National Electrical Code and NFPA 72. The facility failed to implement an approved maintenance and testing program complying with applicable requirements of NFPA 70 and 72. Findings include:
The combination fire and smoke dampers located in the combination fire and smoke walls were not apart of the annual testing and was not followed up with a test when the outside vendor was called out for repair.

Based on observation and interview with staff, the facility failed to include all equipment on a preventive maintenance program in accordance with NFPA 99 chapter 4-5.1.2.10 and chapter 9. Findings include:
The facility failed to include the Bottled Medical Air Gas System and Medical Gas Vacuum System on a preventive maintenance program . The visual warning lamps were out on the medical gas system and the vacuum pump was not on a preventive maintenance program.
