Based upon observations and plan review, the critical access hospital failed to meet the required construction requirements.
Failure to meet the required construction requirements could result in the failure of the construction to contain a fire.
The findings include:
1) Review on 06/09/10 of original construction plans for the 1962 main structure indicate that the exterior walls of the hospital are masonry, and the interior walls and partitions are of combustible, wood-frame construction. This indicates that the most appropriate construction type for this hospital is Type III (211) construction.
2) NFPA 101 (2000 ed) specifies that a hospital of Type III (211) construction requires full protection in all areas by a complete automatic sprinkler system.
3) It was observed during tours of the hospital on 06/09/10 - 06/11/10 that the following two areas were not protected by an automatic sprinkler system:
a) The operating room
b) The combustible-construction overhang at the exit door near the birthing room, which is greater than 4 feet in size.

Failure to provide the required automatic sprinkler coverage could result in the failure of the construction to contain a fire.
The findings include:
1) See Tag K0012 for the requirement that a complete automatic sprinkler system must be installed in the hospital.
2) It was observed during tours of the hospital on 06/09/10 - 06/11/10 that the following two areas were not protected by an automatic sprinkler system:
a) The operating room
b) The combustible-construction overhang at the exit door near the birthing room, which is greater than 4 feet in size.

Based upon observation, the critical access hospital failed to limit trash collections
containers to less than 32 gallon capacity within a 64 square foot area, unless stored in a room protected as a hazardous area.
Failure to limit the size of trash collection receptacles risks a fire of an intensity that
would not be able to be controlled by the light hazard sprinkler system.
Findings include:
During a tour of the hospital on 06/09/10, it was observed that:
1) Two plastic re-cycle wheeled bins of approximately 20 gallons each were stored in the entry way to the medical records office.
2) An additional plastic recycle wheeled bin exceeding 32 gallons in capacity was stored in an open closet in the medical records office.
3) The door into the medical records office does not meet the requirements for a room protected as a hazardous area when not attended, because although the room was equipped with a door closing device, the closer requires a manual action to release from the friction type hold open device. The door is not automatically closing or self closing.
