Based on record review and staff interview, the facility failed to correct deficiencies identified by the automatic sprinkler system contractor on the required annual inspection. This has the potential to affect all patients/ residents, staff, and visitors in all eight smoke compartments.

Findings include:

1. On 7/21/14 at 10: 15 a.m., the records for the automatic sprinkler system were reviewed. The 6/4/12 on up to the 5/22/14 sprinkler reports identified the check valve for the fire department connection was plugged. This blockage would fail to allow the fire department to introduce water from a fire truck if the city water was unavailable.

Staff member A, maintenance supervisor, indicated the check valve had neither been repaired or replaced.
