Based on document review and interview, issues that were identified during a hospital investigation did not have actions for improvement developed or implemented.

Findings include:

-- Per review of facility investigative meeting minutes, the facility identified issues during a code blue event. There was no documentation that the issues identified had action plans developed or implemented. A thorough investigation was not completed.

-- During interview of Staff A, Clinical Staff, on 10/2/2025 at 11:00 am, they confirmed the above finding.
