Based on observation, record review and interview the facility failure to ensure the crash cart and defibrillator were checked daily per hospital policy.
Findings include: The hospital policy revised on 10/17/23 titled "Crash Cart", stated "the House Supervisor will check the crash cart/defibrillator monitor every 24 hours."
Observation on 05/23/2024 at 1109 of the North Unit revealed the crash cart had not been checked on 05/14,5/18, and on 5/19/2024.
Interview with the nurse manager (ID#2) during the observation, verified the findings.
