(A) The smoke wall of Phoenix and Perserverance
(B) Around the window to Pharmacy
(C) The ceiling to the main electrical room and the exterior dietary hot water heater room.

By observation on 1/22/2010 and onward smoke barriers were non-compliant, specific findings include penetrations in the following areas:
(A) Smoke wall to adjacent compartment in dayroom above the ceiling.

By observation on 11/2/09 and onward the following HVAC regulations were non-compliant, specific findings include improper air balance between the corridor and the three bedrooms and conference room in the girls adolescent area. Currently the air flow to the roof based HVAC unit is believed by staff to return warm air from the adjacent area beyond the cross corridor smoke doors causing the unit to not cycle on and provide warm air to the bedrooms and conference area when needed.
