Based on observation, staff interviews, and facility policy review, it was determined that the facility failed to ensure that all visitors are screened for COVID-19 upon entrance to the facility in accordance with facility polices.
Findings include:
Reference: Facility policy titled, "HMH Facility COVID Screening and Surveillance Policy 2.0" states, "...All visitors will be screened for COVID-19 symptoms as described in CDC [Centers for Disease Control and Prevention] Guidelines... All patients entering a clinical facility will be screened for COVID-19 symptoms and exposures. ..."
1. During a tour conducted on 11/8/2021 between 11:05 AM and 11:15 AM, in the presence of Staff #17 in the Emergency Department (ED) lobby, one (1) person, was observed walking through the ED entrance towards Staff #18. The person was not screened for COVID-19.
2. Staff #18 confirmed this finding upon interview, on 11/8/2021 at 11:20 AM.
