Based on medical record review, the facility did not meet the nursing needs of Patient #1 related to skin care.

Findings include:

Review of Inter-Hospital Transfer report dated 1/19/11 revealed "red bottom - skin not open" " inc of urine."

Review of the Patient Care Plan report revealed the patient's skin integrity would be assessed every shift (twice a day). Skin integrity assessment was documented as having been completed at the following times:
1/22/11 - 8:00 AM
1/23/11 - 7:30 AM and 8:00 PM
1/26/11 - 8:20 PM
2/01/11 - 11:00 AM
2/05/11 - 7:30 AM
2/06/11 - 9:30 AM
2/07/11 - 2:40 PM
2/10/11 - 7:30 AM

Review of wound care report dated 2/6/11 revealed redness to sacrum, small amount of skin sloughing, macerated. Very small amount of blood noted. On 2/10/11, a note revealed redness over all prominences.

There is no evidence skin assessments were performed in accordance with the patient care plan.
