Based on observation and staff interview, the facility failed to provide a safe environment to patients housed in six (6) patient rooms on the Adult Behavioral Health Unit.
Findings include:
Observation during a tour of the Adult Behavioral Health Unit on 7/11/16 at 2:00 PM with the CNO revealed six (6 rooms- 667, 675, 677, 683, 684, and 685) contained electric beds with cords approximately eight (8) feet in length.
The CNO acknowledged the above findings, and immediately had the beds replaced with wooded framed beds.
Review of facility policy AMC-RI.030, Patient Rights and Responsibilities, revised September 2014, revealed that Patient Rights include:
(a) Respect and Dignity- the patient has the right to personal dignity, considerate, respectful care; provided in a safe environment free from restraints unless the patient presents a safety risk to himself or herself and/or others.
(b) Patient Safety- Each patient has the right to receive care in an environment that is safe at all times. The patient has the right to receive care that is environmentally and emotionally safe.
