Based on record review, observations, and interviews with facility staff, evidence of Line Isolation Monitors (LIM's) testing could not be provided. Testing of polarity, grounding and ground fault interruption protection is necessary to safeguard patients and staff from the hazards of electric shock and the performance of utilization equipment in accordance with manufacturer ' s requirements.
Findings include:
On 8/20/2015 while conducting records review with the facility Plant Operations Director, no records of the testing of line isolation monitors were provided. The Plant Operations Director said that he was unaware of testing requirements and that two of the three operating rooms were not in use. While on tour of the facility, the operating room used as an endoscopy procedure room was observed to be equipped with a LIM. When the test switch was pushed, the LIM did not respond. The other two LIM's were located, manually tested and had the same results. The According to NFPA 99 (1999 edition) 3-3.3.4.1; "Patient Care Areas. If installed, the isolated power system shall be tested in accordance with 3-3.3.4.2."
