Based on clinical record review and staff interview, the facility failed to ensure respiratory services were provided as per physician orders for 1 of 4 (Patient # 2) sampled patients.
The findings include:
Review of the patient's respiratory care service revealed the patient received several treatments prior to having a documented physician prescription as follows:
a. BiPAP- 09/05/2015 at 4:20 PM - "pt placed on BiPAP 12/5, B/U 14, 35%, per patient uses device as needed at home. mostly at night. at the 9:53 PM assessment, the patient remained on the BiPAP; 09/06/2015 at 8:33 AM, patient on the BiPAP; 12:10 PM and 5:19 PM, remains on the BiPAP. Patient was administered the Oxygenation via the BiPAP machine for 2 days without a supportive physician order prescribing this practice
