Sprinkler System - Supervisory Signals

Based on observation and interview the sprinkler system was not maintained to include tamper switches on control valves that would alarm when sprinkler operation is impaired.

On 4/16/24 at 11:45 AM during a tour of the sprinkler room the control valves for the sprinkler system were observed. These control valves were open at the time of observation and the sprinkler system was operational, but the tamper switch alarm devices were not installed. In the event the control valves were to be closed the system would not provide a signal in accordance with NFPA 72.

These findings were reviewed with Facilities Maintenance and Director of Support Services at the time of observation. Later during the survey, the tamper switches were observed as having been installed.

Subdivision of Building Spaces - Smoke Barrie

Based on observation and interview a smoke compartment wall was not maintained to provide the required ½ hour fire resistance rating and smoke resistance.

Findings include:

On 4/15/24 at 12:20 PM during a tour of the facility areas above the ceiling at fire and smoke rated walls were observed. Unsealed penetrations were observed above the ceiling in the smoke compartment wall located in the central corridor at the western side of the building. Several pipes and wires that passed through the wall were not sealed around the penetrations.

These findings were reviewed with Facilities Maintenance and Director of Support Services at the time of observation. Later during the survey, the penetrations were observed as having been sealed.
