Based on document review and interview, nursing failed to follow policy in reporting a critical lab in 1 of 10 (Patient 1) medical records reviewed.

Findings Include:

1. Review of policy titled: Patient Change in Condition (PC.50) last reviewed 3/2021, indicated provider must be notified of the change in condition, if the provider does not respond within 15 minutes, another call will be placed. If the provider does not respond within 30 minutes, the Chief Medical Officer will be notified.

2. Review of Patient 1's medical record indicated that a Na (sodium) level was drawn on 10/27/21 and resulted at 12:48 am on 10/28/21 with a critical level of 166 (normal is 137 - 145). Physician was called once and nurse left voicemail with patient's lab values and made no other attempts to reach the physician.

3. Interview on 02/07/22 with P50 (Administrator) at 1:40 pm confirmed that nursing did not follow policy by not calling the provider a second time.
