Based on observation and interview, the facility failed to have an exit within thirty feet of the terminal end on the south side of its two corridors thereby potentially increasing time for occupants to locate the one exit access door to the stairway in the smoke compartment. This has the potential to affect all patients and visitors to the facility. The census was 16 patients.
Findings include:
On 05/23/17 at 12:45 PM a tour was taken of the facility with Staff Q and M.
Observation of the wing revealed a door to an exit stair on the western corridor. Approximately 42 feet from the door there was a cross-corridor, and 14 feet (for a total of 56 feet) further the corridor ended at a door to an activity space.
Observation of the eastern corridor revealed no exit access door to the stairway, and it also ended at a door to the same activity space. (14 feet from that door is the same cross-corridor.)
