Based on observation and staff interview, the facility does not maintain the kitchen floor and wall tiles in an intact and safe condition.

Findings include:

Observation during the tour on 7/24/14 at 2:30PM revealed deteriorated floor tiles in the following areas:
-walk-in freezer;
-grease trap cover at 3 bay pot sink;
-cooking line;
-dish machine room.

This tour also revealed broken wall tiles located at the rear kitchen door and dish room.

Based on observation and staff interview, the facility does not maintain the Generator Room in a safe condition.

Findings include:

Observation during the facility tour, on 7/24/14 at 10:40AM, revealed an oil spill under the emergency generator located in the "Generator Room", thereby creating a safety hazard.

This finding was verified with Staff #20 on 7/24/14.

Based on observation, staff interview and document review, the facility does not properly store dental patient care products.

Findings include:

Observation during the facility tour, on 7/24/14 at 11:20AM, revealed the external digital thermometer on the Panasonic dental product refrigerator, which is not a commercial grade refrigerator, read 58 degrees Farenheit. The internal mercury thermometer read 35 degrees Farenheit. Interview with Staff #11 and review of the refrigerator logs revealed that staff had been using the external thermometer with a reading of 58 degrees Farenheit. An accurate thermometer was obtained and confirmed the internal refrigerator temperature was 35 degrees Farenheit.

Dental products in the refrigerator were labeled as requiring storage at varied temperatures, for example:
-Henry Schein Dental X-ray requires storage temperatures between 50 and 75 degrees Farenheit;
-Prime and Bond Elect Bonding Agent requires storage temperatures between 35 and 36 degrees Farenheit;
-Voco Futura Bond U Bonding Agent requires storage temperatures between 39 and 73 degrees Farenheit;
-Henry Schein Etch Gel 40% requires storage temperatures between 50 and 75 degrees Farenheit;
-Alloybond Base/Primer requires storage temperatures between 50 and 68 degrees Farenheit.

This finding was verified with Staff #11 on 7/24/14.

Based on observation and staff interview, the facility does not provide secure medical record storage.

Findings include:

Observation during the facility tour, on 7/24/14 at 11:30AM, revealed numerous boxes of medical records that were stored in two dental equipment machine rooms located in the dental center. These rooms are accessed by equipment contractors who are not supervised by facility personnel and, therefore, the security of the records is not ensured.
