Based on observation the facility did not maintain the facility in compliance with NFPA 101.

Findings Include:

Observation during the tour on 8/11/11 between the hours of 10:30 am and 2:00 PM revealed roller latches on 18 patient room doors on the second and third floor (Room #'s 201, 213, 215, 216, 218, 219, 231, 246, 247, 249,313, 315, 316,318, 335, 346, 347, and 349.)

Observation during the tour on 8/11/11 revealed a dutch door on the nurses charting room (Room # 336.) This dutch door only included the bottom half of the door. The entire opening is required to be protected with an enclosure.

These findings were verified with Staff # 4 on 8/11/11.

Observation during the tour on 8/11/11 between the hours of 10:30 AM and 2:00 PM revealed the furnace room (Room #113) is not protected with a one-hour separation or sprinkler as required by this regulation.
