Based on observation, interview, and record review, the facility failed to ensure patients were free from all forms of abuse as evidenced by:
(1) 3 of 17 patients (Patient #3, #9 and Patient #11) reported that staff members used abusive language toward patients and 1 of 5 staff members (Personnel #64) stated that other staff members had used abusive language toward patients.
(2) 10 of 25 patients (Patients #1, #24, #25, #26, #27, #28, #29, #30, #31 and Patient #32) had personal belongings lost or not returned to them at discharge.

Record review of the Complaint Form by Patient #3 [no date or time] revealed allegations of being cursed by staff during her hospitalization 08/21/2015-09/11/2015.
In interviews with 15 inpatients from the adult psychiatric units (4, 5, and 6) on 12/05/2016 (0955-1115), Patient #9 and Patient #11 stated they had heard a staff member curse a patient. Patient #3 named Personnel #63 (MHT) as the staff member she overheard cursing at patients.
In an interview with Personnel #64 (RN) on 12/05/2016 at 1130, she stated she has heard one staff member curse at patients but did not name the staff member.
In interviews with Personnel #55 (CEO) and Personnel #52 (Director of Quality) on 12/05/2016 at 1430, they stated it is against hospital policy for staff to curse at patients.
Record review of the Patient Rights given to patients at the time of admission [no date or time] revealed: "You have a right to be free from mistreatment, abuse, neglect, and exploitations."
